Millennium Physician Group in Florida seeks a Care Manager to support patients and families by coordinating appointments, care team communication, and community resources. You will navigate insurance basics and payer requirements under moderate supervision.

You will track outreach, resolve scheduling issues, and escalate complex concerns to licensed staff, leveraging strong communication and documentation skills. 2–4 years in healthcare navigation preferred.

Qualifications

• High school diploma or equivalent required; 2-4 years of healthcare navigation, care coordination, medical office, call center, or community resource experience.
• Experience with scheduling and patient communication standards.
• Experience using electronic health record, scheduling, care management, or customer relationship management systems.

Responsibilities

• Coordinate patient access activities including appointment scheduling, referral follow-up, resource connection, and care team communication.
• Review patient information to identify common logistical, financial, social, or operational barriers to care access.
• Resolve moderately non-routine scheduling, documentation, or service coordination issues within established procedures.
• Communicate next steps to patients and families in a clear, empathetic, and culturally responsive manner.
• Track outreach, pending actions, completed tasks, and unresolved barriers in designated systems.
• Partner with clinical, operational, payer, and community resource teams to support timely patient follow-up.
• Escalate clinical concerns, complex access barriers, or situations outside established procedures to licensed clinical staff or leadership.

A Care Manager supports patients and families through routine and moderately complex navigation needs by coordinating appointments, care team communication, community resources, and follow-up activities. Resolves common access barriers, interprets standard workflows, and helps patients understand next steps across care settings, payer requirements, and internal service channels.The Care Manager carries out defined clinical support processes under moderate supervision, applying working knowledge to tasks that occasionally require interpretation or deviation from standard procedures. Decisions are procedure-driven and impact day-to-day patient interactions and team workflow.
