The hours we are hiring for are Monday-Friday or Monday-Thursday until 8pm. If they are full time, it can be five 8hr shifts (12pm-8pm) or four 10hr shifts (10am-8pm). Part time could start as late as 4pm working until 8pm.

Essential Functions

• Participates as an active treatment team member providing crisis services, modeling effective crisis services, and assisting other staff in learning to provide crisis services to individuals in crisis with mental health and substance use issues.
• Engaging and developing rapport with individuals in crisis situations presenting with a variety of symptoms/presentations.
• Demonstrating the ability to recognize and assess client risk of danger to self or others and provide appropriate responses.
• De‑escalating immediate crisis, assessing client needs, and developing treatment plans to address those needs.
• Developing safety and after care plans to reduce future crisis.
• Coordinating and collaborating with community resources to meet client needs.
• Participating in on‑call coverage and responding to on‑call incidents to meet contract requirements and timelines.
• Providing training, modeling, and coaching related to case management and crisis support to new staff members.
• Providing support to supervisors in coordinating activities of case managers to ensure effective coverage and service delivery for clients.
• Acting as agency contact for community partners (when assigned) to promote effective coordination of care.

• Demonstrating the ability to recognize and identify symptoms of mental illness and substance use.
• Participating as a member of a multidisciplinary team approach to treatment and recovery.

Outreach, Engagement, Follow‑Up

• Providing comprehensive outreach and follow‑up to all assigned individuals, including contact attempts over the phone, in‑person, and at home.
• Engaging and developing rapport with individuals presenting with a variety of symptoms/presentations.

• Providing services that are person‑centered, culturally relevant, and non‑judgmental, open, and welcoming to diverse populations.

Documentation

• Documenting all services provided within expected time frames.
• Documenting all services in a manner that meets service definitions, regulatory standards, and agency expectations.
• Documenting all services in a person‑centered manner that meets quality improvement goals.

Efficiency and Productivity

• Demonstrating excellent organizational skills.
• Demonstrating effective use of time management techniques.
• Consistently meeting established productivity standards.

Staff Development Activities

• Participating in and completing all required trainings accurately and timely.

Qualifications

Minimum Education Criteria: High School Diploma or GED with a minimum of 2 years of relevant experience working with the target population.

Experience: Dependent on type of education, as determined by the hiring manager.

• Ability to recognize and identify symptoms of mental illness and suicide and risk potential and respond appropriately.
