Under supervision, you will provide interventions to enhance independent living, assist in developing care plans, and coordinate services across medical and behavioral health disciplines.

Role emphasizes crisis response, suicide risk assessment, bilingual communication, and engagement with diverse populations. Requires advanced degrees or related experience and valid driver’s license.

• Bachelor’s degree in a human service-related field with 1+ year relevant experience
• HS Diploma or GED with 3+ years experience in the target population
• Certified Peer Specialist (CPS) or Certified Family Specialist (CFS) are acceptable

The Care Manager works under the supervision of the Program Manager and Clinical Director/Supervisor.
