Job summary

New Mexico Solutions seeks a Care Manager to deliver person-centered behavioral health services in Albuquerque. Under supervision, you will provide interventions to enhance independent living, assist in developing care plans, and coordinate services across medical and behavioral health disciplines.

Role emphasizes crisis response, suicide risk assessment, bilingual communication, and engagement with diverse populations. Requires advanced degrees or related experience and valid driver’s license.

Qualifications

• Master’s Degree in a human service-related field or
• Bachelor’s degree in a human service-related field with 1+ year relevant experience
• HS Diploma or GED with 3+ years experience in the target population
• Certified Peer Specialist (CPS) or Certified Family Specialist (CFS) are acceptable

Responsibilities

• Provide individual interventions to increase independent living and community participation.
• Develop and support care plans with goals addressing symptoms and barriers.
• Coordinate access to medical, behavioral health, pharmacology, and recovery support services.
• Participate in multidisciplinary team treatment and recovery planning.
• Conduct outreach and engagement with assigned individuals and document services.
• Engage clients with a person-centered, culturally relevant, non-judgmental approach.
• Document all services timely and in compliance with standards; attend supervision and training.

The Care Manager works under the supervision of the Program Manager and Clinical Director/Supervisor.
