Qualifications

• 2+ years of health care experience in care management or related setting.
• Clinical licensure/certification required only if mandated by state contracts.
• Valid and unrestricted driver's license and reliable transportation.
• Knowledge of community resources and local service providers.
• Ability to work independently with minimal supervision and handle multiple tasks.
• Strong verbal and written communication skills.
• Proficiency in Microsoft Office and related software.

Responsibilities

• Complete member assessments within regulated timelines and determine eligibility for care coordination.
• Develop and implement care plans with member, caregiver, physician, and care team.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plan effectiveness and document interventions and goal achievement.
• Maintain an ongoing member caseload with regular outreach.
• Promote integration of behavioral health, LTSS, and home/community resources.
• Facilitate interdisciplinary care team meetings and collaboration.
• Use motivational interviewing to educate and motivate change.
• Identify and address barriers to care through coordination support.
• Collaborate with licensed care managers as needed.
• Travel 25-40% may be required.
