Responsibilities

• Conduct telephonic, in-person, or home visits as required.
• Monitor care plan effectiveness and document interventions and goal achievement.
• Maintain ongoing member caseload and regular outreach.
• Promote integration of services including behavioral health, LTSS, and home/community resources.
• Facilitate interdisciplinary care team meetings and collaboration.
• Use motivational interviewing to educate and motivate change.
• Assess barriers to care and assist members accordingly.
• Collaborate with licensed care managers as needed.
• Travel 25-40% as required by state/contractual requirements.
