Qualifications

• Education: RN or LMSW/LCSW with active Texas license.
• Experience in care/case management, chronic disease management or population health preferred.
• At least two years in care coordination and planning or related area.
• Experience working in interdisciplinary teams.
• Excellent computer skills and proficient in documentation.

Responsibilities

• Provides primary care management interventions to identified members enrolled in Health Management programs.
• Identify members at risk for poor outcomes and coordinate enhanced follow-up and care coordination.
• Develop and execute a comprehensive care plan with providers, payers, and patient/family.
• Provide proactive outreach via telephone, internet, or in-person encounters.
• Facilitate transitions in care and collaborate with post-acute providers and community resources.
• Promote adherence to established care plans and optimize utilization of clinical resources.
