Job summary

Sea Mar Community Health Centers seeks a Care Coordinator to optimize care for patients with chronic conditions and behavioral health needs. You will participate in huddles, identify gaps, and provide point-of-care interventions to improve patient-centered care.

In this role, you will connect patients with Sea Mar and community resources, assist with care transitions, and support ongoing self-management goals. Strong communication and bilingual skills are highly valued.

Qualifications

• LPN with ambulatory care experience and additional health-related degree or training.
• Experience working in community health or ambulatory care settings is preferred.

Responsibilities

• Coordinate care for patients with chronic conditions and behavioral health needs.
• Participate in daily huddles and identify patient needs according to protocols.
• Provide brief interventions at point of care and link patients to behavioral health services.
• Follow up with patients and track adherence to care plans in charts.
• Collaborate with care team to improve quality, care transitions and patient experience.
