Formación

• Graduate of an accredited school of nursing or Bachelor's Degree in Social Work (BSW) required.
• Experience in the clinical or acute care setting preferred.
• LVN/LPN or LBSW in the state of employment is required.

Responsabilidades

• Meets competencies: Leader of Self/Others/Leaders and supports patient care goals.
• Interview patients/families to assess social, emotional and financial factors affecting care.
• Develop and monitor the patient’s plan of care with the care team for effective discharge.
• Coordinate care progression across the continuum and remove discharge barriers.
• Advocate for patients and families regarding treatment decisions and post-acute needs.
• Document care plans and communication to ensure continuity and regulatory compliance.
