Job summary

Monarch Communities is seeking a Care Manager - HHA for a full-time role in New York. The position supports residents with daily living activities, personal care, and social engagement per care plans. You will report to the Health & Wellness Director and work a schedule from Tuesday to Saturday 7am–3pm and Thursday to Monday 3pm–11pm.

The role offers an hourly wage of $20–$22, plus a $500 sign-on bonus paid after 60 days. Benefits include comprehensive health coverage and retirement options.

Qualifications

• Certified HHA required.
• Experience with elderly population preferred; dementia experience helpful.

Responsibilities

• Provide resident care including bathing, dressing, toileting, and feeding as per care plan.
• Maintain safety, comfort, dignity, and confidentiality of residents.
• Escort residents to meals and activities and ensure they are clean and groomed.
• Document services provided during shift and report changes to supervisor.

The Home Health Aide will report to Health & Wellness Director.

Job Overview

Assists residents in all aspects of their daily life as indicated in the resident’s care plan, including personal care, food service, housekeeping, laundry, behavior management, socialization, and activities.

Tuesday - Saturday -7am-3pm

Thursday - Monday -3pm-11pm

Salary Range: $20.00 - $22.00 Hourly

Sign on Bonus; Receive a $250 bonus after completing 30 days of employment and an additional $250 bonus after completing 90 days of employment.

Responsibilities and Duties

• Provide resident care to residents that include bathing, dressing, toileting, and feeding, as outlined in their care plan
• Maintain the safety, comfort, dignity, and confidentiality of all residents
• Escort residents to meals and activities
• Ensures all assigned residents are clean, neatly dressed and groomed, safe, and fed during assigned shifts
• Document all services provided to your assigned residents during your shift
• Report any change of condition in a resident immediately to your supervisor

• Knowledge and experience in working with the elderly population, preferred dementia experience

Physical Abilities

• While performing the duties of this job, the associate is often required to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell.
• An individual in this position will be required to lift or carry weight in up to 50 lbs. Intermittent physical activity includes lifting and supporting residents. The associate must use proper body mechanics.
