Job summary

Monarch Communities, in collaboration with Brandywine Senior Living, is seeking a dedicated Care Manager to provide compassionate personal care and companionship to residents. You will support residents with daily living activities and monitor changes to update care plans, promoting participation in community programs.

The role requires a CNA or HHA certification and a high school diploma; experience with the elderly, including dementia, is preferred.

Qualifications

• Certified Nursing Assistant (CNA) or Certified Home Health Aide (HHA) certification.
• High school diploma or equivalent is required or preferred.
• Knowledge and experience in working with an elderly population; dementia experience preferred.

Responsibilities

• Provide warm, engaging personal care services, support, and companionship to residents.
• Assist residents with activities of daily living (ADLs).
• Monitor changes in residents' day and personal care needs and update their Resident Care Plan.
• Encourage resident participation in community programs and activities.

Full Time Schedule:

(7:00am-3:00pm) Week 1: Monday, Tuesday, Thursday, Friday, Saturday / Week 2: Sunday, Tuesday, Wednesday,Thursday, Friday

Salary Range:

$20.00-$21.00 Hourly

Qualifications

• Certified Nursing Assistant or Certified Home Health Aid
• Knowledge and experience in working with the elderly population, preferred dementia experience

Physical Abilities

• While performing the duties of this job, the associate is often required to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell.
• An individual in this position will be required to lift or carry weight in up to 50 lbs. Intermittent physical activity includes lifting and supporting residents. The associate must use proper body mechanics.
