Job summary

Elwyn is seeking a compassionate TCM Care Manager in Raleigh, NC to serve a caseload of 30-50 members across community-based and office-based settings. You will advocate for needs, coordinate care with healthcare teams, and empower individuals to manage health and medications.

The role requires travel in Wake and Durham Counties, strong organizational skills, and an autonomous, collaborative approach. A driver’s license and EHR proficiency are essential, with a RN or human service degree

Qualifications

• Bachelor's degree in a human service field or RN license is required.
• Minimum 2 years of experience working directly with individuals with behavioral health conditions.
• Ability to travel in Wake and Durham Counties.
• Must possess a current, valid driver's license and have at least two years of driving experience in the United States.

Responsibilities

• Advocate and Navigator: Assess needs, develop care plans addressing physical and behavioral health, medication management, and social determinants.
• Care Coordinator: Facilitate communication between beneficiaries, doctors, specialists, and providers; ensure care plans are followed.
• Empowerment Coach: Educate and empower beneficiaries to manage health, navigate the system, and develop healthy habits.
• Data-Driven Leader: Analyze data to identify improvements and document interactions and progress.
