Crossroads Treatment Centers is seeking a Care Manager in Wilkinsburg, PA to connect with patients monthly, address needs, and coordinate resources. You will collaborate with community partners to support patient access to services and work with a team to deliver high-quality care.

Responsibilities include maintaining partner relationships, promoting Crossroads' MAT approach, and participating in community events to raise awareness. Previous healthcare experience is preferred.

Qualifications

• High School Diploma or GED required.
• Experience in healthcare or addiction treatment preferred.
• Experience with building individualized care plans is a plus.
• Knowledge of compliant documentation practices.
