The Care Manager for the Adult Crisis Stabilization Unit will be responsible for providing advocacy, coordination and continuity of evidence-based care for individuals and families in need through a collaborative process of triage, assessment, planning, and communication. Participates as a member of a multidisciplinary treatment team. Reports to the unit/department supervisor.

QUALIFICATIONS:

PHYSICAL DEMANDS:

Abilities in reaching, bending, talking,, sitting, carrying, standing, grasping, fine hand coordination, ability to hear, ability to read and write, and the ability to remain calm under stress. Ability to reach with hands and arms, climb stairs, balance, stoop, kneel, crouch or crawl. Specific vision abilities include close, distance, color, peripheral, depth perception and the ability to adjust focus. Must be capable of lifting 50 pounds of dead weight alone. May be required to complete standing presentations.

WORK ENVIRONMENT:

High-paced, collaborative team performance with the emphasis on critical thinking and decision-making with guidance from program supervisor/manager. Climate controlled office environments with frequent exposure to clients. Some programs may require traveling within Hillsborough County via personal vehicle or agency vehicle to facilitate in-home client services or onsite clinical programs in a variety of socioeconomic neighborhoods.
