Job summary
Eliot Community Service Agency (CSA) seeks a compassionate Family Partner to support youth and families within our community behavioral health network. You will collaborate with the CSA team, families, and providers to advance care plans and connect families to essential community resources.
The ideal candidate will have an associate’s degree in human services, a valid driver’s license, and experience navigating child/family service systems.
Qualifications
- Associate’s degree in human services or related field.
- Must have a valid driver’s license and reliable transportation.
- Must be a parent, guardian or caregiver of a youth with behavioral health needs.
- Experience in navigating child/family service systems.
- Bilingual candidates fluent in Spanish, Portuguese or Creole eligible for 6% pay increase.
Responsibilities
- Develop measurable family partner goals to support ICP and related goals.
- Engage caregiver(s) in activities at home and in the community to meet ICP goals.
- Assist caregiver(s) in meeting the youth's needs through education, support, coaching, modeling and advocacy.
- Teach caregiver(s) how to network/link to community resources and treatment providers.
- Support caregiver(s) in advocating for services and resources to meet youth and family needs.
- Guide, support and foster empowerment including linkages to peer/parent support.
