Job summary

Eliot Community Service Agency in Danvers, MA, is seeking a Care Manager to coordinate youth and family services across the North Shore. You will implement individualized care planning, conduct home-based assessments, and navigate complex service systems with a collaborative team.

Applicants should have a Bachelor’s degree in human services or related field, a valid driver’s license, and experience in child mental health and crisis management.

Responsibilities

• Serve as key contact for youth and family to Eliot CSA services.
• Initiate care planning activities with all members of the family and/or caregiving system.
• Conduct a comprehensive home-based assessment using the CANS, CRAFTT (for youth 12 and older), SNCD and other tools as needed, in the youth’s home or a location chosen by the family.
• Initiate planning that includes intake meetings, orient the family to the wraparound process, identify strengths and needs of the child.
• Develop and implement a youth and family centered Individualized Care Plan (ICP) in collaboration with the family and collaterals.
• Develop an initial safety/crisis plan.

Eliot’s Community Service Agency (CSA) serves children and youth (through age 21) and their families who are in need of support in managing and coordinating services. Eliot is excited to align these essential CSA services with the catchment area of our established Community Behavioral Health Centers (CBHCs) to offer a seamless and effective community-based continuum of care.

The ideal candidate for the Care Manager - CSA position is a dedicated and collaborative professional with a strong background in child and family behavioral health, crisis management, and care coordination. They possess excellent communication and organizational skills, demonstrate cultural sensitivity, and are committed to empowering families through individualized care planning, system navigation, and ongoing support.

Schedule

Monday - Friday, 9:00am-5:00pm

This position is based in Danvers, MA and requires travel throughout the North Shore and northern Essex County, including Danvers, Beverly, Gloucester, Rockport, Ipswich, Haverhill, Amesbury, Newburyport, and surrounding communities.

Annual Salary: $55,000 USD - $58,000 USD
