Job summary

Eliot Community Service Agency (CSA) seeks a Family Partner to support families navigating behavioral health services for youth. The role emphasizes advocacy, collaboration, and empowering families to access community resources and achieve care plan goals.

The ideal candidate is a caregiver of a youth with behavioral health needs, with strong communication and a drive to improve family outcomes. Schedule is Monday–Friday, 9:00am–5:00pm, hourly rate $26.44–$28.85.

Qualifications

  • Associate’s degree in human services or related field.
  • Must have a valid driver’s license and reliable transportation.
  • Must be a parent, guardian or caregiver of a youth with behavioral health needs.
  • Experience in navigating child/family service systems.
  • Bilingual candidates fluent in Spanish, Portuguese or Creole eligible for 6% pay increase.

Responsibilities

  • Work closely with the CSA team, family, and other collaterals in developing measurable family partner goals to support the family’s ICP and related goals.
  • Engage caregiver(s) in activities in the home and in the community that are designed to address one or more goals/tasks on the family’s ICP.
  • Assist caregiver(s) in meeting the needs of the youth through educating, supporting, coaching, modeling, advocating and guiding.
  • Teach caregiver(s) how to network/link to community resources and treatment providers.
  • Support caregiver(s) in advocating for services and resources to meet youth and family needs.
  • Guide, support and foster empowerment including, linkages to individual, peer/parent support and self‑help groups.

Education

Eliot's Community Service Agency (CSA) serves children and youth (through age 21) and their families who are in need of support in managing and coordinating services. Eliot is excited to align these essential CSA services with the catchment area of our established Community Behavioral Health Centers (CBHCs) to offer a seamless and effective community-based continuum of care.

Family Partner – CSA
