Job summary

AppleGate Recovery is seeking a dedicated Care Coordinator to provide care management services for individuals in recovery and help them build personalized plans for long-term success.

You will work with a compassionate team to connect clients with community resources, document progress, and collaborate with healthcare providers to optimize recovery outcomes.

Qualifications

• High school diploma or GED required.
• Certified Recovery Specialist (CRS) or relevant experience in recovery, peer support, and/or case management preferred.
• Strong communication and organizational skills are essential.
• Passion for helping others achieve recovery goals.
