Job summary

AppleGate Recovery is seeking a Care Coordinator to join our care team in a recovery-focused outpatient clinic. You will provide care management, help clients develop personalized plans, and coordinate access to community resources to support long-term recovery.

Ideal candidates will have a High School Diploma/GED and be CRS certified or able to obtain certification within 6 months, with strong communication and organizational abilities and a passion for helping others achieve recovery goals.

Qualifications

• Certified Recovery Specialist (CRS) or willingness to obtain within 6 months.
