Job summary

Qualifications

• Graduate of an accredited school of LPN or RN with current Vermont licensure.
• BSN or MSN strongly preferred.
• Three years in hospital/clinic setting preferred; utilization/case management experience preferred.
• Certified Case Manager preferred.

Responsibilities

• Collaborate with care team to evaluate patient needs and plan care.
• Coordinate post-discharge planning and community resources.
• Advocate for timely, safe transitions and manage care across settings.
• Maintain accurate patient records and ensure documentation quality.
