Qualifications

• Bachelor's degree in related field preferred.
• Experience in mental health or substance use treatment settings.
• Valid driver's license and reliable transportation.
• Successful completion of a criminal history background check after hire.

Responsibilities

• Network with employers in Rutland County to develop work opportunities aligned with client goals.
• Coordinate care during transitions and build trusting relationships with clients and families.
• Conduct structured interviews and warm hand-offs to clinical teams.
• Administer standardized assessments and enter data into systems.
• Coordinate access to mental health, primary care and community services.
