Monarch Communities is seeking a caring Care Manager to deliver warm, compassionate personal care and companionship to residents. You will assist with daily living activities and help update care plans based on observed changes in residents’ needs.

Ideal candidates have a CNA or HHA certification and a high school diploma or equivalent, with a positive attitude and a passion for helping others in a community setting. Full-time, on-site role with growth opportunities and benefits.

Qualifications

• High school diploma or equivalent is required.
• CNA or HHA certification is required.

Responsibilities

• Provide warm, engaging personal care services, support and companionship to residents.
• Assist residents with activities of daily living (ADLs).
• Monitor and update personal Resident Care Plans based on observed needs.
• Encourage participation in community programs and activities.
