Job summary

Your Health Organization in Myrtle Beach is seeking a Clinical Coordinator to assist with all clinical needs for our patients. The role includes chart preparation, scheduling patient visits, coordinating and logging chronic care management activities, and providing provider and patient support.

This full-time, salary-based position runs 8:00 AM to 5:00 PM, Monday through Friday, with daily presence in the Myrtle Beach office.

Qualifications

• Must be CMA or licensed nurse in the state of employment.
• Minimum 1 year experience in a healthcare setting.
• Must hold and maintain current CPR certificate.
• High school diploma or equivalent.
• Strong interpersonal and communication skills.
• Ability to read and communicate effectively.
• Basic computer knowledge.
• Willingness to travel between offices.

Responsibilities

• Review and organize patient medical records in preparation for visits.
• Ensure all necessary documentation and results are accessible for the provider.
• Verify consent forms and alignment paperwork are in the chart.
• Reconcile medications.
• Schedule and coordinate patient appointments with providers and care team.
• Interact with patients via email, portal, phone, text, or in person to schedule and instruct.
• Use Navina, AthenaText, and Teams for communication and updates.
• Document and log care management activities accurately.
