• Knowledge of recovery, harm reduction, and wellness-based concepts.
• Proficiency in Microsoft Office Suite and similar platforms.
• Proficiency in using electronic health record systems and remote collaboration tools (Teams, Zoom).
• Strong communication, organizational, and time-management skills.

Responsibilities

• Administer psychosocial screenings/assessments.
• Coordinate linkage to behavioral health treatment, medical, educational, and vocational services as needed.
• Provide ongoing professional care management services for the justice-involved population.
