Job summary

Coastal Horizons Center is seeking a TASC Care Manager to support justice-involved individuals across Hertford, Northampton, and Gates Counties. You will administer psychosocial screenings and coordinate access to behavioral health, medical, educational, and vocational services.

The role requires strong communication, organization, and ability to manage a caseload independently. The position is based in North Carolina with full-time hours, typical M-F schedule, and comprehensive benefits.

Qualifications

• Bachelor's degree in human services or related field required.
• Preference for NC Peer Support Specialist certification or NCASPPB registration with 2+ years experience.
• Experience with recovery, harm reduction, and wellness concepts.

Responsibilities

• Administer psychosocial screenings/assessments.
• Coordinate linkage to behavioral health, medical, educational and vocational services.
• Provide ongoing care management for justice-involved individuals.
• Collaborate with community partners, justice systems, and service providers.
