Job summary

Coastal Horizons Center is hiring a TASC Care Manager in Washington, NC to support justice-involved individuals through psychosocial assessments and service coordination. Primary duties include linking clients to behavioral health treatment and medical, educational, and vocational services, with a focus on recovery and harm-reduction principles.

The role requires strong communication, organization, and time-management skills, plus experience with EHR systems and remote collaboration tools.

Qualifications

• Bachelor's Degree or higher in a human services or related field.
• Experience or certification in NC peer support or addiction services is advantageous.
• Knowledge of recovery, harm reduction, and wellness-based concepts.

Responsibilities

• Administer psychosocial screenings/assessments.
• Coordinate linkage to behavioral health treatment and other services.
• Provide ongoing care management for justice-involved individuals within TASC program.
