Job summary

RHA Health Services, LLC in Wake County, North Carolina, is hiring a Care Manager for adult and child behavioral health. This hybrid position involves delivering high-quality integrated care management across sites, homes, and communities, traveling within and around the county.

The role supports individuals with mental health, substance use, and intellectual/developmental disabilities, coordinating comprehensive assessments, care planning, and data-driven follow-up.

Qualifications

• For all populations served, meet one QP qualification: master’s in a human service field AND 1 year of MH/SU/IDD experience.
• Or bachelor’s in human services with 2+ years of MH/SU/IDD experience; or bachelor’s in non-human services with 4+ years MH/SU/IDD experience.
• LTSS/HCBS coordination experience preferred; care management experience required.

Responsibilities

• Care Management Service Delivery: assess, plan, and monitor care with the person supported and team.
• Follow policies and documentation standards for whole-person care.
• Collaborate cross-functionally with Integrated Care Team and providers.
• Maintain person-centered approach focusing on strengths and needs.
• Use data to support integrated, well-coordinated care management.
