Job summary

RHA Health Services, LLC is hiring a Behavioral Health Care Manager for Craven County. This hybrid, community-based role delivers integrated care management across sites, homes, and communities with emphasis on physical and behavioral health for adults and children.

The position requires QP qualifications with minimum degrees and MH/SU/IDD experience; valid driver’s license is needed. Travel within Craven County is expected and scheduling offers flexibility.

Qualifications

• Master’s degree in a human service field with at least 1 year of MH/SU/IDD experience.
• Bachelor’s degree in a human service field with at least 2 years of MH/SU/IDD experience.
• Bachelor’s degree in a non-human service field with at least 4 years of MH/SU/IDD experience.
• Two years of LTSS/HCBS coordination or care management experience.
• Valid driver’s license required; clinical license preferred.

Responsibilities

• Care management service delivery and coordination.
• Conducts comprehensive assessments and develops tailored care plans with the person supported and the team.
• Monitors and updates care plans at least annually or as needs change.
• Collaborates with Integrated Care Team and community partners to implement care management.
• Maintains documentation, ethics, and compliance; ensures data protections and quality standards.
• Promotes person-centered care and continuous quality improvement.
• Fosters cross-functional collaboration and effective communication with stakeholders.
