Job summary

RHA Health Services is seeking an IDD Care Manager for Burke, Buncombe and McDowell counties with a $3,000 sign-on bonus. This hybrid, community-based role requires travel within and around the county and focuses on integrated care across physical/behavioral health, I/DD, and related services.

The position emphasizes comprehensive assessments, tailored care planning, and ongoing plan updates, collaborating with integrated care teams to promote person-centered outcomes and high-quality care.

Qualifications

• Care managers must meet at least ONE of the qualifications to meet the QP criteria per 10A NCAC 27 .0104.
• Master’s degree in a human service field with at least one year of full-time MH/SU/IDD experience.
• Bachelor’s degree in a human service field with at least two years MH/SU/IDD experience.
• Bachelor’s degree in a non-human service field with at least four years MH/SU/IDD experience.
• Two years of LTSS/HCBS coordination or care management experience.
• Care managers should have care coordination, care management or case management experience.
• Valid driver’s license; clinical license preferred; reliable transportation.

Responsibilities

• Care Management Service Delivery – Fosters and facilitates the care management process.
• Conducts Care Management Comprehensive Assessments, shares information and facilitates Tailored Care Management Planning with the person supported and other team members.
• Monitors the ongoing care management plan implementation and facilitates Care Plan Updates at least yearly or when there is significant change.
• Implements workflows, policies and procedures, and documentation standards that promote whole person care.
• Maintains a positive and professional image in the work setting and community.
• Follows policies and procedures to ensure accountability of service delivery.
• Best Practices – Stays updated on best practices and informs the team about innovations.
• Cross-functional Collaboration - Collaborates with Integrated Care Team members, community orgs and departments.
• Person-Centered Approach – Maintains a person-centered philosophy and educates on self-management and integrated planning.
• Quality Assurance and Use Of Data - Uses data to support integrated care delivery.
• Maintains a Safe and Healthy Environment; ensures health and safety policies.
• Financial Performance - documents services timely for reimbursement; maintains external relationships.
• Ethics & Compliance – supports ethical culture and investigations according to policy.

Education

Master’s degree in a human service field

Bachelor’s degree in a human service field

Bachelor’s degree in a non-human service field

At least one year of full-time experience with MH/SU/IDD

Two years LTSS and/or HCBS coordination experience

Care coordination, care management or case management experience

Valid driver’s license

Clinical license preferred
