Qualifications

• Two years of experience with individuals having behavioral health, I/DD, or TBI conditions.
• Prior experience in LTSS and/or HCBS coordination and care management for those with LTSS needs.

Responsibilities

• Conduct comprehensive assessments and care planning.
• Coordinate care with team members and monitor service engagement.
• Educate clients on care options and promote customer satisfaction.
• Ensure compliance with service orders and maintain documentation.

Skills

Proficiency in Person-Centered Thinking/planning

Experience using assessments to develop plans of care

Knowledge of LOC processes

Familiarity with Medicaid benefits plans

Proficiency in Motivational Interviewing

Strong communication skills

Conflict management and resolution skills

Proficiency in Microsoft Office

Good organizational skills

Education

Bachelor's degree in health, psychology, sociology, social work, nursing or another relevant area
