Job summary

RHA Health Services, LLC is hiring a Care Manager for Adult Behavioral Health in West Buncombe County. The role focuses on integrated, person-centered care across sites and communities with a strong emphasis on collaboration with the integrated care team.

Hybrid/remote work is available with travel within the county. The ideal candidate will have RN or related human service qualifications and experience in MH/SU/IDD, with opportunities for advancement and a $3,000 sign-on bonus for full-time

Qualifications

• RN and four years of MH/SU/IDD experience or
• Master’s in human services with at least one year MH/SU/IDD experience
• Bachelor’s in human services with at least two years MH/SU/IDD experience
• Bachelor’s in non-human service field with at least four years MH/SU/IDD experience

Responsibilities

• Provide whole person, integrated care management across sites, homes and communities.
• Coordinate care planning with persons supported and the care team.
• Conduct ongoing assessments and updates to care plans at least annually.
• Ensure compliance with policies, procedures and documentation standards.
