Job summary

Santiam Hospital & Clinics seeks an experienced Care Manager to lead patient-centered discharge planning from admission through transition of care. You will identify high-risk patients, coordinate post-acute services, and collaborate with multidisciplinary teams to ensure safe, timely transitions and reduced readmissions.

The ideal candidate will have strong communication, critical thinking, and decision-making skills, with experience in care transitions and community resources.

Qualifications

  • RN or LPN or MSW/LCSW required.
  • Experience in care management, discharge planning, case management, or utilization management.
  • Strong understanding of care transitions and post-acute care services.
  • Excellent communication, collaboration, and critical thinking skills.

Responsibilities

  • Lead comprehensive, patient-centered discharge planning from admission through transition of care.
  • Identify high-risk patients early and implement proactive discharge strategies.
  • Coordinate post-acute services and community resources including home health, SNF, rehab, hospice, and outpatient care.

As a Care Manager, you will play a critical role in delivering patient-centered discharge planning and care coordination across the continuum of care. This position partners with patients, families, providers, and interdisciplinary teams to ensure safe, timely, and well-coordinated transitions from hospital to post-acute settings, with a strong focus on reducing avoidable readmissions and improving patient outcomes.The ideal candidate will possess strong communication skills, exceptional critical thinking skills, and the ability to make sound decisions to support patients through a successful transition of care.Apply today and bring your expertise to Santiam Hospital & Clinics, recently honored as 2025’s Best Hospital in the Willamette Valley!What You’ll DoFull-time (32 hours per week) position leading comprehensive, patient-centered discharge planning beginning at admission and continuing through transition of care.Identify high-risk patients early and implement proactive strategies to support safe discharge planning and reduce adverse outcomesPerform holistic assessments of clinical needs, functional status, support systems, home environment, and social determinants of health (SDOH)Develop and manage individualized discharge plans aligned with patient goals, clinical needs, and available resourcesCoordinate post-acute services and community resources, including home health, skilled nursing, rehabilitation, hospice, and outpatient careProactively identify and resolve barriers to discharge, including placement challenges, insurance authorization, equipment needs, and social or financial concernsSupport informed patient choice by educating patients and families on post-acute care options and available servicesFacilitate seamless care transitions, ensuring timely communication with post-acute providers, outpatient teams, and community partnersParticipate in multidisciplinary rounds, providing clear updates on discharge readiness, barriers, and next stepsEnsure accurate, timely, and compliant documentation of assessments, plans, and discharge outcomesCoordinate durable medical equipment (DME), follow-up care, and appointments prior to discharge to promote continuity of careSupport patient and caregiver education, reinforcing discharge instructions and promoting understanding of care plansMonitor utilization and length of stay, supporting efficient throughput and appropriate level-of-care decisionsWhat You’ll NeedRequired:One of the following:Licensed Registered Nurse (RN)Licensed Practical Nurse (LPN)Master of Social Work (MSW) or Licensed Clinical Social Worker (LCSW)Experience in care management, discharge planning, case management, or utilization managementStrong understanding of care transitions, post-acute care services, and community resourcesExcellent communication, collaboration, and critical thinking skillsAbility to manage multiple priorities in a fast-paced acute care environmentPreferred:Prior experience in acute care case management or hospital-based care coordinationCase Management certification (e.g., CCM, ACM)Experience working with complex patients, high-risk populations, or social determinants of health barriersFamiliarity with regulatory and accreditation standards related to discharge planning and transitions of careBenefits for Eligible PositionsMedical, Vision and Dental InsurancePTO and holiday payEmployee Referral Program401(k) RetirementLife InsuranceLong Term DisabilityEmployee DiscountsBilingual Pay Differential for eligible positionsPublic Service Loan Forgiveness for eligible positionsTuition Assistance for eligible positionsWhy Santiam Hospital?At Santiam Hospital & Clinics, we pride ourselves on fostering a supportive and inclusive work environment where every team member is valued and empowered to make a difference.
