Job summary

Community Care, Inc. in Wisconsin is seeking a Case Manager to join our mission-driven team serving older adults and adults with disabilities in Sheboygan County.

You will manage an assigned caseload with an RN Case Manager, complete assessments and care plans, coordinate services, and document to standards. The role offers a 5,000 sign-on bonus and a Monday–Friday schedule. Experience with long-term care screening is a plus, and local travel is required.

Qualifications

• Bachelor’s degree in a Human Services or closely related area is required.
• Minimum of three years’ experience with seniors or adults with disabilities may substitute for a non-human services degree.
• One year of experience with older adults or persons with physical or intellectual disabilities is preferred.
• Eligibility for long-term care functional screener certification is required.
• Regular local travel with a valid driver’s license and adequate auto insurance is required.

Responsibilities

Case Manager Job Requirements

• Education: Bachelor’s Degree in a Human Services or closely related area required. A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.
• Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred.
• Certification: Eligibility for long-term care functional screener certification is required
• Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Case Manager Job Schedule

Monday to Friday, 8:00 AM-4:30 PM
