Job summary

Direction-Home-of-Eastern-Ohi is seeking a qualified Care Manager to coordinate home and community-based services in Kentucky. The role requires RN or LSW licensure and at least one year of relevant experience.

You will manage data, maintain documentation, coordinate with providers, educate consumers about resources, and ensure compliance with HIPAA and agency policies while traveling within the four-county service area.

Qualifications

• Must be a Registered Nurse (RN) or Licensed Social Worker (LSW) with current, unencumbered license.
• At least one year of experience in home health care, medical social work and/or geriatrics.
• Must possess a valid driver’s license and proof of automobile insurance.
• Must pass a criminal background check and drug screen.

Responsibilities

Summary of Duties and Responsibilities:Under the general supervision of the Director of Home and Community Based Services direct supervision of the direct supervisor, the Care Manager (CM) is responsible to abide by CS program contractual expectations. The CS CM is expected to manage and coordinate comprehensive services and assure that they are appropriate and meet the needs of the consumer to support them.Responsibilities:Will be able to add and manage data in Excel.Computer knowledge of M Fax, Microsoft Outlook, and Microsoft Office.Demonstrate ability to create and organize member consumer files.Demonstrates ability to be flexible to the changing environment of ODA/ODM and Vendors to support the Agency in meeting goals.Educate consumers about community resources available when a consumer is disenrolled from the programEnsure timely reporting to all mandated agencies as per Ohio law.Working knowledge ofMedicare, Medicaid and third-party reimbursementCommunity service delivery systems and the ability to link consumers to community resources.Maintains minimum proficiency in all requisite computer applications and software.Participate in and contribute toward accomplishment of divisional and Agency goals.Maintains timeframes for timely and accurate documentation in the appropriate databases.Maintain consumer and provider confidentiality according to HIPAA and Agency policy.Participates in training to maintain and improve job skills.Interprets and consistently applies the Agency’s Policies and Procedures.Promotes positive employee relations and fosters excellent customer service.Perform duties with respectful communications and behaviors.Perform other duties as assigned.Must have the following abilities to perform the essential functions:Ability to effectively communicate with the general public, in person, by phone, and via email.Ability to comprehend written material related to essential job functions.Ability to accurately document facts related to essential job functions.Ability to comprehend, recall, and apply facts related to essential job functions.Ability to analyze, evaluate, and implement a reasonable course of action based on available information.Ability to lift/carry 20 lbs. upstairs.Ability to travel to community and individual locations within our four-county service area (Ashtabula, Mahoning, Trumbull and Columbiana counties).Ability to occasionally travel outside of our four-county service area including but not limited to Columbus, Ohio.Ability to climb steps and access private homes/locations which may not be readily accessible.Minimum Job Qualifications:Applicant must be a Registered Nurse or Licensed Social Worker with current, unencumbered license, with at least one year’s prior experience in home health care, medical social work and/or geriatrics.Demonstrates working knowledge and skills necessary to provide the assessment and care management required by the consumer.Applicant must possess a valid driver’s license and proof of automobile insurance.Must also pass a criminal background check and drug screen.
