Job summary

Direction Home Of Eastern Ohio is hiring a Care Manager to coordinate home and community based services in the Youngstown area. The CM reports to the supervisor and ensures services meet consumer needs while complying with program requirements.

Minimum qualifications include being a Registered Nurse or Licensed Social Worker with an active license and at least one year in home health care, medical social work, or geriatrics. Valid driver's license and background check required.

Qualifications

• Must hold a valid RN or LSW license with at least one year of relevant home health or geriatrics experience.
• Ability to assess consumer needs and coordinate services per contractual expectations.
• Proficiency with Excel and standard office software for data management and reporting.

Responsibilities

• Coordinate comprehensive home and community based services for consumers.
• Manage and organize consumer files and documentation timely and accurately.
• Educate consumers about available community resources and Medicare/Medicaid information.
• Maintain confidentiality according to HIPAA and Agency policy.
• Communicate with stakeholders by phone, email, and in person regarding service delivery.
• Travel within four-county area and possibly to Columbus as needed.

Job Details: Job Location: Main Office - Youngstown, OH, Salary Range: Undisclosed, Summary of Duties and Responsibilities:

Under the general supervision of the Director of Home and Community Based Services direct supervision of the direct supervisor, the Care Manager (CM) is responsible to abide by CS program contractual expectations. The CS CM is expected to manage and coordinate comprehensive services and assure that they are appropriate and meet the needs of the consumer to support them.

Responsibilities:

• Will be able to add and manage data in Excel.
• Computer knowledge of M Fax, Microsoft Outlook, and Microsoft Office.
• Demonstrate ability to create and organize member consumer files.
• Demonstrates ability to be flexible to the changing environment of ODA/ODM and Vendors to support the Agency in meeting goals.
• Educate consumers about community resources available when a consumer is disenrolled from the program
• Ensure timely reporting to all mandated agencies as per Ohio law.
• Working knowledge of
  • Medicare, Medicaid and third-party reimbursement
  • Community service delivery systems and the ability to link consumers to community resources.
• Maintains minimum proficiency in all requisite computer applications and software.
• Participate in and contribute toward accomplishment of divisional and Agency goals.
• Maintains timeframes for timely and accurate documentation in the appropriate databases.
• Maintain consumer and provider confidentiality according to HIPAA and Agency policy.
• Participates in training to maintain and improve job skills.
• Interprets and consistently applies the Agency’s Policies and Procedures.
• Promotes positive employee relations and fosters excellent customer service.
• Perform duties with respectful communications and behaviors.
• Perform other duties as assigned.

• Ability to occasionally travel outside of our four-county service area including but not limited to Columbus, Ohio.
• Ability to climb steps and access private homes/locations which may not be readily accessible.

Qualifications:

Minimum Job Qualifications:

• Applicant must be a Registered Nurse or Licensed Social Worker with current, unencumbered license, with at least one year’s prior experience in home health care, medical social work and/or geriatrics.
• Demonstrates working knowledge and skills necessary to provide the assessment and care management required by the consumer.

Applicant must possess a valid driver’s license and proof of automobile insurance.

Must also pass a criminal background check and drug screen.
