Job summary

Qualifications

• High School diploma or equivalent required.
• Associates or advanced degree preferred.
• Two years administrative experience required in a health care setting.

Responsibilities

• Greets visitors and health care personnel promptly and professionally; screens visitors and telephone calls to determine urgency of contact and to facilitate appropriate routing or referral.
• Effectively communicates clinical information with payer to obtain authorization for post-acute services/medications and documents interactions in patient’s EMR.
• Participates in discharge planning activities to ensure timely patient discharge and linkage with post-acute providers.
• Provides feedback regarding payer determinations to streamline the current patient plan of care.
• Function as placement specialist to research preferred providers for home health, DME, acute rehab, SNF, LTACH.
• Communicates payer information changes to supervisor and documents appropriately.
• Verifies with patient financial counseling that all payment sources are explored and documented in EMR.
• Participates in reimbursement, certification and authorization activities.
• Coordinates with referral agencies to arrange provision of equipment and services as directed by Care Managers.
• Collects and transmits relevant clinical and demographic information for post-discharge arrangements.
• Coordinates transportation and community services as directed by care managers.
• Provides clerical support including faxing, copying, filing, typing and scanning.
• Maintains confidentiality and ensures patient and payer satisfaction through professional communication.
