Job summary

AdventHealth in Orlando, FL is seeking a Discharge Planning Manager to oversee daily operations of discharge planning and social services across inpatient, observation, and emergency department settings, serving as the primary contact for barriers and concerns.

You will implement Care Management strategies, participate in multidisciplinary rounds, and drive effective transition planning with facility leadership.

Qualifications

• Knowledge of medical necessity for appropriate patient status and level of care.
• Knowledge of CMS Conditions of Participation for discharge planning and related guidelines.
• Ability to lead others effectively while maintaining compliance.
• Ability to manage multiple tasks and prioritize effectively.
• Strong communication and collaboration across interdisciplinary teams.

Responsibilities

• Supervises daily operations of discharge planning and social services across inpatient, observation, and ED.
• Implements Care Management strategies in collaboration with facility leadership.
• Participates in rounds to ensure effective discharge planning and barrier resolution.
• Leads care conferences for complex transitions and placements as needed.
• Uses reports and dashboards to monitor documentation accuracy and regulatory compliance.
