Job summary

AdventHealth in Orlando, FL is seeking a full-time Discharge Planning Supervisor to lead inpatient discharge and social services. You will drive interdisciplinary rounds, collaborate with facility management, and ensure timely, appropriate patient progression of care. Strong CMS knowledge and data analytics are required.

Preferred BSW with MSW, and 1+ year in care management. Travel and onboarding support may be available as part of the AdventHealth team.

Qualifications

• BSW required; MSW preferred.
• 1+ year direct care management experience; 2+ years in social work.
• ACM/CCM certifications preferred.

Responsibilities

• Supervises daily discharge planning and social services; serves as primary contact for barriers.
• Implements Care Management strategies with facility management.
• Participates in multidisciplinary rounds and complex transition planning.
• Provides expertise in discharge planning and progression of care to the team.
• Uses reports/dashboards to monitor care manager documentation and regulatory compliance.
• Facilitates care conferences for complex transitions and placements.
• Participates in meetings to review high-cost or long-stay cases.
