Job summary

ChristianaCare is seeking an energetic Social Worker to join our care management team in Delaware. The role emphasizes care coordination, psycho-social support, and discharge planning within an integrated care model.

Eligible candidates have an MSW/MSS and state licensure (CSW/MSW) with acute care experience preferred; sign-on bonus of up to $15,000 is available for a two-year commitment.

Qualifications

• Master’s degree in Social Work (MSW) or Master of Social Services degree (MSS) from an accredited graduate school.
• Licensed CSW or MSW as required by the state of Delaware prior to hire date.
• Acute Care Social Work experience preferred.

Responsibilities

• Collaborate with RN Case Manager and Utilization Management to drive discharge planning.
• Resolve psycho-social barriers and support patient discharge needs.
• Participate in care coordination across the care management triad.
