Job summary

ChristianaCare in Delaware is seeking a Social Worker with a strong focus on care coordination and discharge planning within a multidisciplinary team. The role supports patient progression and collaborates with RN Case Managers and Utilization Management to ensure appropriate care and timely discharge.

The position requires an MSW or MSS, and licensure as CSW or MSW per Delaware rules. Acute care experience is preferred. A sign-on bonus is available for up to $15,000 with a two-year commitment.

Qualifications

• MSW or MSS degree required from an accredited graduate school.
• Licensed CSW or MSW as required by the state of Delaware.

Responsibilities

• RN Case Manager – manage patient care and discharge planning.
• Social Worker – resolve psycho-social barriers and supports discharge needs.
• Utilization Management – review patient status for payer appropriateness.
