Job summary

ChristianaCare is seeking an energetic social worker to join our care management team in the Delaware region. The role focuses on care coordination, discharge planning, and addressing psycho-social barriers to ensure safe transitions of care.

Applicants must hold a Master’s in Social Work (MSW or MSS) and be licensed as CSW or MSW in Delaware. Acute care experience is preferred; a sign-on bonus may be available and comprehensive benefits are provided.

Qualifications

• Master’s degree in Social Work (MSW) or MSS required prior to hire date.
• Licensed CSW or MSW as required by the state of Delaware prior to hire date.
• Acute Care Social Work experience preferred.

Responsibilities

• RN Case Manager – manage patient care and drive patient progression and establish a discharge plan.
• Social Worker – resolve psycho-social barriers and supports discharge needs.
• Utilization Management – review patient status for appropriateness and anticipated payer coverage.

Education

MSW or MSS from an accredited graduate school

Licensed CSW or MSW as required by the state of Delaware
