El Rio Health is seeking a Registered Nurse Care Coordinator to promote high-quality nursing care and patient safety. The role involves disease management, referral coordination, and post-discharge follow-up for high-risk patients, with emphasis on social determinants of health and patient engagement.

The RN will lead care coordination workflows, monitor outcomes, and support the care team in delivering patient-centered services within a community health setting in Tucson, AZ.

Qualifications

• RN license in good standing from the Arizona Board of Nursing.
• Current Basic Life Support (BLS) certification.
• Level I fingerprint clearance card with ability to meet driving requirements.
• Reliable transportation with valid driver’s license and auto insurance.
• One year of hands-on patient care nursing experience in a hospital or clinic setting.

Responsabilités

• Identifies patients for care coordination through risk stratification and referrals.
• Engages patients in care coordination.
• Implements interventions based on risk stratification and evidence-based guidelines.
• Develops a comprehensive plan of care with patient/family involvement.
• Monitors patient/family response and revises plan of care as needed.
• Provides self-management support and education to patients and families.
• Coordinates care with providers, patients, and community resources.
• Performs medication management and reconciles medications.
• Maintains up-to-date community resources and documentation.
• Follows up after transitions between care settings and ensures timely care.
