Eonhealth in Denver, Colorado is seeking a Navigator I to join our Care Management team in a hybrid role. You will engage patients and providers at key moments, coordinate follow-up care, and support scheduling and adherence to care pathways using the EPM platform.

This non-clinical, administrative role emphasizes empathy, clear communication, and operational accuracy. You will document interactions, follow scripts, escalate complex inquiries, and collaborate with internal teams to uphold data

Qualifications

• Associate degree or higher in a healthcare-related field preferred.
• Foundational knowledge of medical terminology required.
• Healthcare experience preferred (2+ years).
• Proficiency with Apple MacBook and Google Suite preferred.

Responsibilities

• Conduct structured outbound calls to coordinate follow-up care.
• Handle inbound calls with empathy and professionalism.
• Help patients overcome logistical barriers to care (scheduling, next steps).
• Follow standardized call scripts and workflows for quality and compliance.
• Accurately document interactions and update patient records in the EPM platform.
• Identify discrepancies and escalate complex inquiries to providers or supervisors.
• Collaborate with internal teams to maintain data accuracy and efficiency.
• Support process improvement and quality assurance initiatives.
