Job summary

CVS Health in Ohio is seeking a Case Management Coordinator for the OhioRISE program. This full-time, field-based telework role requires travel within Western Ohio up to 50% and scheduling flexibility to meet member needs.

You will build care plans, coordinate with multidisciplinary teams, and advocate for high-quality, coordinated behavioral health services for children and families. The position includes a 40-hour work week, with potential after-hours as needed and comprehensive benefits.

Qualifications

• 2+ years in behavioral health or related human services.
• 2+ years of experience with MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years in children’s mental health, child welfare, or related public sector field.
• 2+ years in one or more of: family systems, case management, child development, or related areas.
• 2+ years experience with Ohio delivery systems and local networks.

Responsibilities

• Provide care coordination and education to members.
• Evaluate needs and tailor care plans.
• Collaborate with care teams and families.
• Monitor progress and document in management tools.
• Escalate quality of care issues and coordinate services.
