PacificSource is seeking a Care Manager Clinician to enhance client wellness through advocacy, assessment, planning and service coordination. You will link clients with providers and resources across the health continuum, ensuring privacy and quality care while applying evidence-based practices.

The role requires an active RN license or equivalent licensure (LPC/LMFT/LCSW) and CCM certification by hire or within two years.

Qualifications

• Knowledge of health insurance and state mandated benefits.
• Experience in case management practice including advocacy, assessment, planning, communication, education, resource management and service facilitation.
• Ability to work with diverse health issues and advocate for clients.

Responsabilités

• Advocate for client wellness and autonomy through assessment, planning and advocacy.
• Coordinate care with providers and services across the health continuum.
• Maintain client privacy, confidentiality and health records security.
• Interact with clients, payers, providers and internal teams to assure quality service.
