PacificSource Health Plans seeks an experienced Clinician Care Manager to support members with comprehensive case management, advocating for access to quality, affordable care. You will assess, plan, coordinate and monitor services in collaboration with clinicians, providers and payers to optimize outcomes and cost-effectiveness.

Minimum of three years of clinical case management experience and an active RN, LPC, LMFT or LCSW credential is required; CCM certification is preferred or obtained

Qualifications

• Minimum of three years of clinical case management experience.
• Active, unrestricted RN license or relevant LPC/LMFT/LCSW credential.
• CCM certification required at time of hire or within two years.
• Knowledge of health insurance and state mandated benefits.

Responsabilités

• Assess, plan, implement, coordinate, monitor and evaluate client care plans.
• Collaborate with clinicians, providers and payers to optimize health outcomes and cost-effectiveness.
• Maintain patient privacy, HIPAA compliance and ethical standards.
