Qualifications

• Minimum of three (3) years of clinical experience, including case management.
• Active, unrestricted RN, LPC, LMFT, or LCSW credential required; CCM certification required at hire or within two years.
• Knowledge of health insurance and state mandated benefits; experience in case management including advocacy, assessment, planning, communication, resource management and service facilitation.
• HIPAA privacy laws and confidentiality requirements; ability to interact with providers, payers, and members to ensure quality care.

• Facilitate client wellness and autonomy through advocacy, assessment, planning, communication, education, resource management, and service facilitation.
• Collect and assess patient information to develop individualized care management plans for appropriate utilization and cost-effective care.
• Coordinate with service providers to link clients with appropriate resources while ensuring safe, effective, client-centered care.
• Maintain privacy and confidentiality of health information and adhere to ethical, legal, accreditation, certification, and regulatory standards.
• Engage with PacificSource personnel to ensure quality customer service and provide medical or contract interpretation as needed.
• Promote effective written and verbal communication; utilize evidence-based tools; apply lean methodologies for continuous improvement.
