Job summary

PacificSource seeks a Clinician Care Manager in Oregon to assess, plan, coordinate, and monitor health and human service needs for members. You will advocate for patients, develop individualized plans, and coordinate with providers, payers, and care teams to ensure safe, effective, and equitable care.

Minimum three years of clinical case management experience plus an RN, LPC, LMFT, or LCSW credential and CCM certification by hire or within two years.

Qualifications

• Active, unrestricted RN license or equivalent professional license (LPC/LMFT/LCSW) required.
• CCM certification required at hire or within two years.
• Minimum three years of clinical case management experience; insurance knowledge.

Responsibilities

• Facilitate client wellness and autonomy through advocacy, assessment, planning, communication, education, and service facilitation.
• Collect and assess patient information to develop a comprehensive, individualized care plan.
• Link clients with providers and resources across the continuum of health and human services.
• Maintain privacy and confidentiality in line with HIPAA and regulatory standards.
• Interact with PacificSource personnel to assure quality customer service.
