PacificSource Health Plans is seeking a Clinician Care Manager to assess, plan, coordinate, monitor, and evaluate health and human service needs for members. The role emphasizes advocacy, communication, and resource management to promote quality and cost-effective care.

The ideal candidate will have an active RN license (or equivalent) and a CCM credential or plan to obtain CCMC within two years. Travel is minimal and HIPAA compliance is essential.

Formación

• Active RN license or equivalent clinical license with relevant certifications.
• Minimum of three years of clinical case management experience is required.
• CCM certification or CCMC timeline within two years of hire.

Responsabilidades

• Coordinate care plans to meet members health and service needs.
• Advocate, educate, and connect members with appropriate resources.
• Collaborate with providers, payers, and employers to optimize outcomes.
• Maintain patient privacy and comply with HIPAA regulations.
• Use EHR and data systems to document assessments and plans.
