Piedmont Cartersville is seeking a Care Management Assistant to support the discharge process within the inpatient and ED settings. You will work with the Care Manager and Social Worker to help patients and families overcome non-clinical barriers to discharge.

The role requires two years of healthcare or social services experience, and education includes a GED or high school diploma with preference for an associate degree in a health-related field.

Qualifications

• H.S. diploma or GED required.
• Associate’s degree or two years of college in a human services/health-related discipline preferred.
• 2 years of healthcare or social services experience required

Work Schedule - Saturday and Sunday are required (the other days of the week are flexible).

Responsibilities

As a member of the multidisciplinary team, the Care Management Assistant provides discharge planning support to the Care Manager and Social Worker and supports patients and families / caregivers by reducing non-clinical barriers to discharge in the Inpatient and ED environments.
