Job summary

Piedmont Cartersville is seeking a Care Management Assistant to join a multidisciplinary team. You will support discharge planning alongside the Care Manager and Social Worker, helping patients and families navigate non-clinical barriers during inpatient and ED stays.

The role requires a high school diploma or GED with preferred related college coursework or an Associate’s degree, plus at least 2 years of healthcare or social services experience.

Qualifications

• Associate’s degree or two years of college in a human services/health-related field preferred.

Responsibilities

As a member of the multidisciplinary team, the Care Management Assistant provides discharge planning support to the Care Manager and Social Worker and supports patients and families / caregivers by reducing non-clinical barriers to discharge in the Inpatient and ED environments.
